Below are the forms needed for Boys Camp at Camp Copass. Every counselor is required to complete the Counselor Screening Form. Every camper is required to complete the Camp Copass Health Form.  Every Camper attending the Challenger Camp program is required to complete the Camp Copass Challenger Camp Release Form.

Each church needs to complete the Camp Copass Registration form and e-mail or send to Camp Copass. Each church also needs to complete the Church Registration Form and e-mail or send to Stanley Smith.
